Scrip Order Form A form developed with the most common scrips ordered by FAST customers. You may also submit your orders on a plain 8 1/2" by 11" paper. Full payment must accompany order -- checks are preferred! |
All orders should be given to Sandy Hopkins by 9:00pm on the dates listed or left in the file box scrip folder. If you cannot bring your order at this time, please contact Sandy Hopkins directly.

This list gives any rules which pertain to a particular scrip. For example some gift cards can be used to pay off charge balances and some can't. Some cards start to lose value after a 24 month period. |
